eHealth
The GPpartners eHealth team works closely with healthcare providers across our catchment on a range of eHealth initiatives. Our goal is to build the capacity of healthcare providers to improve their eHealth capabilities. We aim to provide support to improve business processes, connectivity and systems integration between healthcare providers to facilitate timely sharing of patient information which will ultimately improve patient health outcomes.
General practice
- Health Record eXchange - a shared electronic health record system.
- Secure messaging - connecting local health practitioners with secure messaging software.
- Canning Data Extraction Tool - a clinical data extraction tool for the most popular GP software applications used to help better target services.
- Training - regular low cost software training for Medical Director, Pracsoft and others + we arrange training for services such as Medclaims and ACIR Online.
- Templates - downloadable commonly used templates.
Hospitals
- Enterprise Discharge Summary (EDS) - Queensland Health computerised patient discharge summary. GPpartners works with the EDS team and GPs to ensure that summaries can be distributed electronically and securely to general practices registered with Queensland Health.
- eReferral - GPpartners is working with Royal Children's Hospital to establish pathways and referral tools to support electronic and secure transmission of referrals and delivery of electronic discharge summaries (EDS).
- iHealthcare Directory - working with General Practice Queensland and the Queensland divisions network to provide a concise healthcare provider service directory for direct patient care purposes.
- GP Register - an electronic listing of GP contact details for GP’s working the GPpartners division.
Health Record eXchange
GPpartners’ Health Record eXchange (HRX) is an electronic health record that allows GPs, hospitals and allied health professionals to share important patient information in a secure system. Shared electronic health records have benefits for all patients but are especially useful for patients with chronic and complex conditions or people who require care from of a number of health professionals. The HRX ensures accurate and timely access to critical patient information by all members of a patient's care team. The HRX can improve patient care by:
- reducing errors
- ensuring a patient’s health information is available to all registered health care providers 24 hours a day
- sharing summary information such as; conditions, medications, allergies, and diagnostic results
- helping healthcare providers better coordinate care for patients
- improving access to patient information across the health sector, regardless of the point of care.
Since 2004 GPpartners has been working with GPs and other health professionals in the north Brisbane area to implement the HRX. The HRX integrates with existing clinical software and is accessible via a secure internet connection. The HRX continues to grow and is currently used to support patient care. Strengthening Electronic Communication Capabilities
The goal of the Strengthening Electronic Communication Capabilities project is to improve electronic communication strategies between healthcare providers in the Northside Health Service District with the aim to improve patient care and management.The purpose of this project is:
- to provide the capacity to strengthen communication strategies which will improve patient care and management
- to implement an electronic communication system which provides secure, effective and timely communication of patient information between multidisciplinary multi sector care teams
- to engage with all members of the North Brisbane Partnership Council to implement a secure electronic communication infrastructure between all participating healthcare providers in the Northside Health Service District
- grow the existing number of healthcare providers using secure electronic communication methods
- identify, connect and train healthcare providers not currently using the Medical Objects secure messaging service.
GPpartners' projects under this initiative include roll out of Medical Objects messaging software system to local general practitioners, specialists and allied health providers.
The project is part of the Connecting Health Care in Communities initiative funded by Queensland Health in partnership with General Practice Queensland, the Queensland Aboriginal and Islander Health Council and the Combined Health Agency Group - Blue Care, Ozcare, RSL Care, Spiritus.
