Community and Hospital Integration
Our Community and Hospital Integration program is about establishing partnerships within local primary and tertiary health care systems to improve delivery of health care services in the community. These partnerships enable us to work on areas of mutual benefit and raise awareness of each other’s issues across health sectors.

GP Advisors work with teams (called Collaboratives) of public and private hospitals executive and staff, meeting regularly to progress initiatives that improve communication between hospitals and general practice including referral, discharge and clinical pathways.

Similarly, Collaboratives with primary health care services within our catchment aim to improve communication between primary care services and general practice and raise awareness of services available and referral pathways.
